Tyler's Place AZ Inc. is a family-owned agency based in Chandler, AZ, licensed throughout Arizona, seeking Direct Care Workers to aid individuals with developmental disabilities at home and in the community. We offer flexible hours, competitive pay, and training provided in a supportive environment.

Responsibilities include respite, attendant care, and habilitation. Candidates must pass AZ DPS Level 1 clearance, hold a valid AZ driver’s license, and ideally CPR/First Aid or related certificates.

Qualifications

• Must PASS AZ DPS Level 1 Fingerprint Clearance application & Criminal History check — State of Arizona requirement
• Valid Arizona Driver's License with clean driving history — State of Arizona Requirement (Willing to transport members)
• CPR & First Aid Certification (Preferred)
• Article 9 Certificate (Preferred)
• Direct Care Worker Certificate (Preferred)
• Prevention & Support Certificate
• 18+ years of age

Responsibilities

• Respite (RSP): supervise and care for Tyler's Place AZ members' physical, mental and/or emotional wellbeing in the member's home or in their community while relieving a family member or person caring for that member.
• Attendant Care (ATC): assist Tyler's Place AZ member in creating/maintaining a safe, clean-living environment which may include personal cleanliness; help engage in community outings and daily plans.
• Habilitation (HAH): assist member in ISP-goal based interventions to improve independence and daily life; may include behavior interventions and/or sensory/motor support.

As a rapidly growing agency we are currently seeking providers to work with individuals with developmental disabilities in the home and the community. As a Direct Care Worker, you will be aiding and support to all age groups by helping to meet their individual goals. We work in a fun and easy-going environment while maintaining a strong work ethic. Responsibilities and Duties

Respite (RSP): The qualified person will supervise and care for Tyler's Place AZ members physical, mental and/or emotional wellbeing in the members home or in their community while relieving a family member or person caring for that member.

Attendant Care (ATC): The qualified person will assist Tyler's Place AZ member in creating/maintaining a safe, clean-living environment which may/may not include personal cleanliness. Provider will also assist member to engage in community outings and/or activities and to help develop and maintain a daily schedule and/or daily plan of care.

Habilitation (HAH): The qualified person will assist Tyler's Place AZ member in a variety of interventions based on the members specific ISP goals to help them become more independent by improving their daily life and to be a valued member of his/her community. Interventions may/may not include Habilitation therapies, behavior interventions and/or sensory motor implantation.
