Job summary

Qualifications

• High school diploma, GED, or equivalent.
• One year of experience in health- or community service-related field.
• Excellent computer knowledge and skills (Microsoft Office).
• Experience in simple data analysis.
• Effective oral, written, and interpersonal communication skills.
• Proficiency in English.

Responsibilities

• Provide patient training on computer/mobile usage for access to medical information and MyChart.
• Provide patient training on Remote Patient Monitoring (RPM) device setup and use.
• Develop patient-facing materials on digital health skills and distribute to patients.
• Distribute information to patients on affordable internet connectivity programs and assist enrollment.
• Advocate for patients by addressing barriers to care (language, health literacy, digital literacy, transportation, insurance, etc.).
• Assist patients with accessing medical, behavioral health, community, and dental services.
• Attend staff and vendor-led technology trainings and weekly status meetings.
• Assist staff in developing evidence-based interventions to increase MyChart activation and evaluate project objectives.
• Performs other duties as needed.
