Job summary

Family Health Centers at NYU Langone is seeking a full-time Digital Navigator in Brooklyn to support the CARE Tech initiative. The role focuses on training patients to use digital tools for health information and MyChart access, and on setting up remote patient monitoring devices.

The position emphasizes creating patient-friendly materials and assisting enrollment in affordable internet programs, while addressing barriers to care and coordinating with clinic staff.

Qualifications

• High school diploma or GED required; 1 year of health- or community-service experience preferred.
• Strong computer skills and experience with data analysis.
• Excellent written and verbal communication; ability to work with diverse populations.
• English proficiency; Spanish and/or Mandarin preferred.

Responsibilities

• Provide patient training on computer/mobile usage for MyChart access.
• Train patients on Remote Patient Monitoring device setup.
• Create patient-facing digital health materials (infographics, brochures).
• Inform patients about affordable internet programs and assist enrollment.
• Advocate for patients by addressing barriers to care (language, literacy, access).
• Assist patients in accessing medical, behavioral health, community, and dental services.
• Attend scheduled technology trainings and weekly status meetings.
• Support development of interventions to increase MyChart activation.
• Support evaluation of project objectives.
• Perform other duties as needed.
