Job summary

Family Health Centers at NYU Langone in Brooklyn is seeking a full-time Digital Navigator to support patients in accessing digital health tools, including training on MyChart and remote patient monitoring. The role reports to CARE Tech leadership and collaborates with the Center for Community-Oriented Virtual Primary Care.

Under supervision, you will develop patient-facing materials, assist enrollment in internet programs, and address barriers such as language and health literacy.

Qualifications

• High school diploma or equivalent required with 1 year of relevant health/community service experience.
• Excellent computer knowledge and Microsoft Office skills, strong data analysis ability.
• Effective oral, written, and interpersonal communication; empathetic and reliable.
• Fluency in English; Spanish and/or Mandarin preferred.

Responsibilities

• Provide patient training on computer/mobile usage for MyChart access.
• Provide patient training on Remote Patient Monitoring (RPM) device setup and use.
• Develop patient-facing materials on digital health skills for distribution.
• Share information on affordable internet programs and assist enrollment.
• Advocate for patients by addressing language, literacy, and access barriers.
• Assist patients with accessing medical, behavioral health, community, and dental services.
• Attend staff and vendor-led technology trainings as scheduled.
• Attend weekly status meetings with site coordinator and CARE Tech team.
• Assist staff in developing interventions to increase MyChart activation.
• Assist staff in evaluating project objectives and outcomes.
• Perform other duties as needed.
