Job summary

Family Health Centers at NYU Langone is seeking a Digital Navigator to join the CARE Tech program in Brooklyn. The Digital Navigator will support patient access to digital health tools and medical information.

You will train patients to use MyChart and RPM devices, create patient-focused materials, and help enroll families in digital connectivity programs. The role requires strong communication, empathy, and bilingual skills where possible.

Qualifications

• High school diploma or GED required; 1 year in health or community service preferred.
• Strong computer skills including Microsoft Office.
• Excellent spoken and written English; Spanish or Mandarin preferred.

Responsibilities

• Provide patient training on using MyChart and Remote Patient Monitoring devices.
• Develop patient-facing digital health materials (infographics, brochures).
• Distribute information on affordable internet programs and assist enrollment.
• Advocate for patients by addressing barriers to care (language, literacy, access).
• Assist patients with accessing a full range of medical, behavioral health, community, and dental services.
• Attend staff and vendor-led technology trainings as scheduled.
• Attend weekly status meetings with site coordinators and CARE Tech team.
• Assist staff in developing and evaluating interventions and project objectives.
• Perform other duties as needed.
