Job summary

Family Health Centers at NYU Langone is seeking a full-time Digital Navigator to support patients in Brooklyn. The role reports to the CARE Tech team and focuses on helping patients access medical, behavioral health, dental services, and digital health tools like MyChart.

Responsibilities include training patients on technology, RPM setup, developing educational materials, and addressing barriers to care. Travel across South Brooklyn is expected.

Qualifications

• Experience in health or community service settings.
• Excellent oral and written communication in English.
• Fluency in Spanish and/or Mandarin.

Responsibilities

• Provide patient training on computer/mobile usage for access to medical and community information, including MyChart.
• Assist patients with Remote Patient Monitoring (RPM) device setup and use.
• Develop patient-facing materials on digital health skills (infographics, brochures).
• Inform patients about affordable internet programs and assist enrollment via digital platforms.
• Advocate for patients by addressing barriers to care (language, health literacy, digital literacy, transportation, insurance, etc.).
• Assist patients with accessing medical, behavioral health, community, and dental services.
• Attend staff and vendor-led technology trainings as scheduled; participate in weekly status meetings.
• Assist CARE Tech staff in increasing MyChart activation at pilot clinics and evaluating project objectives.
• Perform other duties as needed.
