Job summary

Family Health Centers at NYU Langone is seeking a Project Facilitator - Brooklyn to support digital navigation and education within CARE Tech. The role trains patients to access medical information online, assists with RPM setup, and develops patient materials for digital health literacy.

Travel within South Brooklyn may be required; strong communication and empathy are essential in engaging diverse communities and addressing social determinants affecting access to care.

Qualifications

• High school diploma, GED, or equivalent.
• One year of health- or community service-related experience.
• Excellent computer knowledge and skills (Microsoft Office).
• Experience in simple data analysis.
• Effective oral, written, and interpersonal communication skills.
• Empathetic, reliable, resourceful, and a strong desire to help people.
• Proficiency in English.

Responsibilities

• Provide patient training on computer/mobile usage for access to medical and community information, including MyChart usage.
• Provide patient training on Remote Patient Monitoring (RPM) device setup and use.
• Develop patient-facing materials (infographics, brochures) on digital health skills and distribute them as needed.
• Distribute information to patients on affordable internet connectivity programs; assist with enrollment via digital platforms.
• Advocate for patients by addressing barriers to care (language, health literacy, digital literacy, transportation, insurance, childcare, etc.).
• Assist patients with accessing medical, behavioral health, community, and dental services.
• Attend staff and vendor-led technology trainings as scheduled.
• Attend weekly status meetings with site coordinators and CARE Tech team.
• Assist staff in developing interventions to increase MyChart activation at pilot sites.
• Assist staff in evaluation of project objectives.
• Performs other duties as needed.
