Job summary

Concerto Renal Services seeks a PRN Social Worker in Massachusetts to provide psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in skilled nursing or rehab facilities. Responsibilities

• Conduct psychosocial assessments and develop individualized care plans with the interdisciplinary team.
• Provides counseling and education on dialysis and advance directives; supports treatment adherence and quality of life.
• Leads monthly care plan meetings with patients and clinical staff; coordinates with facility teams and regional managers.
• Maintains EMR documentation and ensures CMS compliance.
