Concerto Renal Services is seeking a PRN Social Worker to join its interdisciplinary team, providing psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in Skilled Nursing or rehab facilities. You will conduct assessments, develop individualized care plans, and ensure CMS guidelines are followed.

• Experience conducting psychosocial assessments.
• Familiarity with CMS guidelines and QAPI standards.
• Ability to document in EMR accurately.

Responsabilités

• Conduct psychosocial assessments, 90-day and annual reviews, and care planning with the team.
• Provide counseling and education on dialysis, transplant options, and advance directives.
• Lead care plan meetings with patients and clinical staff; support new clinic openings and admissions.
• Maintain EMR documentation and ensure CMS compliance.
