Job summary

Qualifications

• Master’s degree in social work is required.
• Must hold a valid state license; clinical not required.
• Unencumbered driver’s license required.
• Intermediate computer skills with MS Word/Excel/Outlook; adapt to Concerto applications.

Responsibilities

• Conduct psychosocial assessments and care planning.
• Provide counseling and education on dialysis, transplantation, and directives.
• Lead monthly care plan meetings with patients and clinical staff.
• Maintain EMR documentation and CMS guideline compliance.
• Coordinate with community resources and discharge planning.
