Concerto Renal Services in Easton, MD is seeking a PRN Social Worker to provide psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in Skilled Nursing or rehab facilities.

You will conduct psychosocial assessments, develop individualized care plans with the interdisciplinary team, and lead monthly care plan meetings while maintaining thorough EMR documentation per CMS guidelines and Concerto policies.

Formación

• Master's degree in social work and active state license.
• Healthcare or dialysis background preferred.
• Unencumbered driver’s license and computer proficiency.

Responsabilidades

• Patient Assessment & Care Planning: psychosocial assessments and care plans with the interdisciplary team.
• Therapeutic Support & Education: counseling on dialysis, transplantation, and advance directives.
• Interdisciplinary Collaboration: lead monthly care plan meetings and coordinate with facility teams.
• Documentation & Compliance: EMR entries and CMS-guided documentation.
• Advocacy & Resource Coordination: connect patients with community resources and discharge planning.
