Job summary

Concerto Renal Services is seeking a PRN Social Worker to perform psychosocial evaluations, counseling, and quality-of-life support for dialysis patients in Skilled Nursing or rehab facilities. You will ensure care planning per CMS guidelines and maintain detailed documentation as part of an interdisciplinary team.

You will report to the market Team Lead and indirectly to the Director of Social Services, supporting new clinic openings, admissions, and advocacy for patient resources.

Qualifications

• Psychosocial evaluations and support for dialysis patients.
• Documentation of assessments and care plans in EMR per CMS guidelines.
• Interdisciplinary collaboration to support patient care and discharge planning.

Responsibilities

• Conduct psychosocial assessments (initial, 90-day, annual).
• Provide counseling and education on dialysis and advance directives.
• Lead monthly care plan meetings with clinical staff and regional managers.
• Maintain EMR documentation and track productivity/outcomes.
