Concerto Renal Services in Kentucky is seeking a Social Worker to provide psychosocial evaluations, counseling, and quality‑of‑life support to dialysis patients in Skilled Nursing or rehab facilities.

You will collaborate with the interdisciplinary team to ensure timely care planning per CMS guidelines and maintain thorough documentation to meet QAPI standards. The role reports to the market Team Lead and is travel‑ready.

Formación

• Psychosocial evaluations, counseling, and quality‑of‑life support for dialysis patients.
• Documentation to CMS guidelines and Concerto policies.
• Interdisciplinary collaboration with care teams.

Responsabilidades

• Conduct psychosocial assessments and develop individualized care plans.
• Provide counseling and education on dialysis, transplant options, and advance directives.
• Lead monthly care plan meetings with patients and clinical staff.
• Maintain accurate EMR documentation and CMS/QA reporting.
• Advocate for patients and coordinate outpatient dialysis placement and community resources.
