Job summary

Concerto Renal Services in the Greater Peoria Area is seeking a PRN Social Worker to deliver psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in Skilled Nursing or rehab facilities.

You will perform assessments, coordinate with the interdisciplinary team, ensure CMS guideline adherence and QAPI standards, and document comprehensively in EMR while aiding discharge planning and community resources.

Qualifications

• Master’s degree in social work required.
• Healthcare or dialysis background preferred.
• Must hold a valid state license; clinical not required.
• Unencumbered driver’s license.
• Proficiency with MS Word, Excel, Outlook and Concerto applications within 60 days.

Responsibilities

• Conduct psychosocial assessments (initial, 90-day, annual, as-needed).
• Administer quality-of-life and depression screenings (KDQOL, PHQ-2/9).
• Develop individualized care plans with the interdisciplinary team.
• Provide counseling and education on dialysis, transplantation options, and advance directives.
