Job summary

Fresenius Medical Care is seeking an entry-level Master of Social Work to provide psychosocial services to dialysis patients, both in-center and home dialysis where applicable. You will assess psychosocial status, participate in care planning with the healthcare team, and support patients and families to improve quality of life.

Under supervision, you will educate patients about transplantation options, coordinate referrals to community resources, and assist with advance directives.

Qualifications

• Masters in Social Work.
• Must have state required license.
• Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed

Responsibilities

• Assess patients' psychosocial status, strengths and needs as part of comprehensive patient assessment.
• Participate in care planning with patient and healthcare team to identify effective interventions.
• Provide psychosocial support and counseling to address non-adherence and quality of life concerns.
• Educate patients about transplantation options and coordinate referrals to community resources.
