Job summary

Fresenius Medical Care seeks an entry level Master of Social Work to provide psychosocial services to in-center and home dialysis patients. You will assess needs, counsel families, and collaborate with the interdisciplinary team to promote rehabilitation, adherence, and quality of life.

In this role you will educate patients about treatment options, connect them with community resources, and support staff with policy and patient education while documenting care activities according to FMCNA

Qualifications

• 0–2 years related experience preferred.
• Masters in Social Work with state licensure requirement.
• Experience collaborating with interdisciplinary healthcare teams is a plus.

Responsibilities

• Assess patients' psychosocial status, strengths and needs to support rehabilitation and treatment goals.
• Participate in care planning with patients and healthcare team to improve quality of life.
• Provide counseling and educational resources to patients and families about dialysis and transplant options.
• Refer patients to community resources and support services to facilitate access to care.
• Document MSW interactions and interventions per policy.
