Job summary

You will assess psychosocial needs, counsel patients and families, and collaborate with the interdisciplinary team to promote rehabilitation and quality of life.

The role emphasizes education on transplant options, utilization of resources, and adherence to policy while documenting care interventions.

Qualifications

• MSW degree required and state licensure to practice.
• 0–2 years of related experience preferred.
• Meets the applicable scope of practice and licensure requirements in the state of employment.

Responsibilities

• Assess patients’ psychosocial status and develop care plans with the team.
• Provide counseling to patients and families.
• Educate patients on transplant referrals and available resources.
