Job summary

Dialyze Direct is seeking a Registered Nurse to support in-center and home dialysis programs in the Cincinnati area. The role emphasizes high-quality patient care, independent critical thinking, and collaborative teamwork with nephrologists and facility staff.

Responsibilities include nursing care, data collection, care planning, documentation, and occasional travel to nearby locations. A valid RN license and BLS certification are required; 6 months RN experience and 3 months hemodialysis

Qualifications

• BLS certification (American Red Cross or American Heart Association) required.
• Ability to lift up to 35 pounds.

Responsibilities

• Provide nursing care, training, and home support with minimal oversight.
• Collaborate with Dialyze Direct team, nursing home staff, and nephrologists.
• Record patient health data during pre-, intra-, and post-hemodialysis.
• Develop and implement a plan of care to achieve outcomes.
• Document interventions in EMR and manage lab collections.

Schedule:

• Full Time
• Fixed schedule/no on call

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• In the state of Texas, 6 months of hemodialysis experience within the last 24 months
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
