Job summary

Dialyze Direct is seeking a Registered Nurse to provide high-quality nursing care and home support for patients undergoing dialysis treatments in Ohio. The role emphasizes careful data recording, collaborative care planning, and adherence to safety standards.

The ideal candidate holds a valid RN license with at least 6 months of RN experience and 3 months of hemodialysis experience, plus BLS certification. Expect a Monday–Friday schedule, day shifts, no on-call, and occasional travel.

Qualifications

• Valid RN license.
• 6 months of RN experience preferred.
• 3 months of hemodialysis experience.
• Texas: 6 months hemodialysis in last 24 months requirement.
• BLS certification required; ability to lift up to 35 pounds.

Responsibilities

• Provide nursing care, training, and home support with minimal oversight.
• Collaborate with Dialyze Direct team, nursing home facility reps, and nephrologists.
• Record patient health data before/after hemodialysis treatment.
• Create and maintain care plans to achieve expected outcomes.
• Document interventions in EMR and perform lab collection/shipping tasks.
• Handle clerical duties alongside patient care (filing, inventory, charts).
• Maintain a positive, respectful attitude toward patients and staff.
• Occasionally travel to nearby locations.
• Administer medications and document administration.

Schedule:

• Monday - Friday Schedule
• Full Time
• Day shift only
• Fixed schedule/no on call

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• In the state of Texas, 6 months of hemodialysis experience within the last 24 months
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
