Job summary

Dialyze Direct is seeking a Registered Nurse to join our Bronx team. This full-time role emphasizes high-quality dialysis care, patient-focused outcomes, and collaboration with nephrologists and facility staff.

The position requires an active RN license, 6 months RN experience preferred, and a background in hemodialysis. A fixed weekday schedule with day shifts and no on-call is provided, with travel to nearby locations as needed.

Qualifications

• Must be a registered nurse with a valid license.
• Experience with hemodialysis is preferred.
• Ability to lift up to 35 pounds and pass color vision testing.

Responsibilities

• Provide nursing care and home support with minimal supervision.
• Collaborate with Dialyze Direct team, facility representatives, and nephrologists.
• Record patient health data before and after hemodialysis treatment.
• Develop and maintain a care plan to achieve expected outcomes.
• Record interventions in EMR to document patient progress.
• Perform lab collection, processing, and reschedule missed collections.
• Handle clerical tasks such as filing and chart creation.
• Maintain a positive, respectful attitude toward patients and staff.
• Occasionally travel to nearby locations.
• Administer medications and document administration.

Schedule:

• Monday - Friday Schedule
• Full Time
• Day shift only
• Fixed schedule/no on call

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• In the state of Texas, 6 months of hemodialysis experience within the last 24 months
• BLS certification (American Red Cross or American Heart Association Required); and
• Successful passing of an Ishihara color vision test;
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position

Compensation:

Salary Range $40-$60/hr. Based on experience.

Compensation offers are based on education, experience, skills, and geographic region.
