Job summary

Dialyze Direct in Grove City, PA is hiring a Full-Time Registered Nurse for day shifts with 25% travel. The nurse will provide focused dialysis care, collaborate with facility teams and nephrologists, and document data in the EMR during hemodialysis.

The ideal candidate holds an RN license and BLS, with 6 months of RN experience preferred and 3 months in hemodialysis; weight for lifting and safety sensitivity are noted. Opportunities for education and advancement are offered.

Qualifications

• Valid RN license.
• 6 months RN experience strongly preferred.
• 3 months of hemodialysis experience.
• In Texas, 6 months of hemodialysis experience within the last 24 months.
• BLS certification (American Red Cross or American Heart Association) required.
• Successful passing of an Ishihara color vision test.
• Ability to lift up to 35 pounds.
• This is a safety sensitive position.

Responsibilities

• Provide nursing care with minimal supervision and support home care needs.
• Collaborate with Dialyze Direct team members, facility representatives, and nephrologists.
• Collect and record health data before/during/after hemodialysis.
• Develop and maintain a care plan to reach expected outcomes.
• Document interventions in the EMR to track progress.
• Perform lab collection, processing, and rescheduling as needed.
• Handle clerical tasks (filing, inventory, admissions, chart creation).
• Maintain a positive, respectful attitude toward patients and staff.
• Occasional travel to nearby locations.
• Administer medications and document administration.

Schedule:

• Full Time
• Day shift only

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• In the state of Texas, 6 months of hemodialysis experience within the last 24 months
• BLS certification (American Red Cross or American Heart Association Required); and
• Successful passing of an Ishihara color vision test;
