Job summary

Dialyze Direct is seeking a Registered Nurse in Pennsylvania to deliver high-quality dialysis care with minimal supervision. You will collect data, implement care plans, and coordinate with physicians and facility staff in a busy setting.

The role requires an active RN license, dialysis experience, and BLS certification. This is a full-time, on-site position with some travel and a strong emphasis on safety and patient outcomes.

Qualifications

• Valid RN license
• 6 months RN experience strongly preferred
• 3 months hemodialysis experience
• In Texas: 6 months hemodialysis experience within last 24 months
• BLS certification required
• Ability to lift up to 35 pounds
• Safety sensitive position

Responsibilities

• Provide nursing care, training, and home support with minimal oversight
• Collaborate with Dialyze Direct team members and facility representatives
• Collect and record patient health data during hemodialysis
• Develop and maintain a care plan to attain outcomes
• Record interventions in EMR
• Perform lab collection, processing, and shipping as needed
• Handle clerical tasks and patient charting
• Maintain a positive attitude toward patients and colleagues
• Occasional travel to nearby locations
• Medication administration and documentation

Schedule:

• Full Time
• Fixed schedule/no on call

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• In the state of Texas, 6 months of hemodialysis experience within the last 24 months
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
