Job summary

Dialyze Direct is seeking a dedicated Registered Nurse to provide dialysis care and patient support in an on-site setting in Virginia. The role emphasizes high-level nursing care, documentation, and collaboration with nephrology teams and facility staff.

The RN should have an active VA license, 6 months of RN experience preferred, and 3 months of hemodialysis experience; BLS certification is required. The position may involve occasional travel and physical tasks up to 35 pounds.

Qualifications

Responsibilities

• Provide nursing care, training, and home support as needed with minimal supervision.
• Collaborate with Dialyze Direct team, SNF facility staff, and nephrologists.
• Record patient data during and after hemodialysis in EMR.
• Develop, implement and adjust care plans to achieve expected outcomes.
• Perform lab collection, processing, and shipping and reschedule missed labs.
• Handle clerical tasks such as filing, inventory, and chart creation.
• Maintain a courteous, professional attitude with patients and staff.
• Occasionally travel to nearby locations; administer medications and document accordingly.

Schedule:

• Full Time
• Fixed schedule/no on call

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• In the state of Texas, 6 months of hemodialysis experience within the last 24 months
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
