Job summary

Fresenius Medical Care Concord, NH is seeking an entry level Professional Registered Nurse in the Outpatient CAP 1 program. This role provides and coordinates clinically competent care, including assessment, planning, intervention and evaluation for an assigned group of patients undergoing hemodialysis.

You will collaborate with RNs, LVN/LPNs and Patient Care Technicians, participate in education and quality improvement activities, and support emergency response when needed.

Qualifications

• Must be a graduate of an accredited School of Nursing and hold a current state nursing license.
• BLS/CPR certification required.
• Entry level RN with less than 2 years nephrology nursing experience.

Responsibilities

• Provide and coordinate clinically competent care for patients undergoing hemodialysis.
• Document patient data and adjust the treatment plan in collaboration with the care team.
• Educate patients and families on treatment options and care plans.
• Administer prescribed medications and assist with emergency response as needed.
• Delegate tasks to RNs, LVN/LPNs and Patient Care Technicians as appropriate.
