Fresenius Medical Care is seeking an Outpatient RN CAP 2 to join the team in Denver, CO. This role provides and coordinates clinically competent care for an assigned group of patients undergoing hemodialysis.

The position advances through the Clinical Advancement Program and may involve teaching, leadership, and participation in quality improvement projects. A current RN license and CPR/BLS certification are required, with experience in nephrology preferred.

Qualifikationen

• RN CAP 2 entry level pathway or advancement from RN CAP 1.
• Accountable for assessment, planning, intervention, and evaluation for an assigned group of patients.
• May delegate tasks to direct patient care staff including RNs, LVN/LPNs, and Patient Care Technicians.
• Member of the ESKD health care team participating in decision‑making, teaching, leadership and quality improvement.

Aufgaben

• Performs essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses.
• Performs ongoing collection and analysis of patient data during hemodialysis and documents in the medical record; adjusts the treatment plan as indicated and notifies appropriate leaders or physicians as needed.
• Assesses, collaborates, and documents patient/family learning needs to provide initial and ongoing education.
• Directs and provides, in collaboration with staff, safe and effective delivery of hemodialysis therapy to assigned patients.
• Administers medications as prescribed or per approved algorithms and documents justification and effectiveness.
• Initiates or assists with emergency response measures.
• Serves as a resource, leader, coach, mentor and role model for new and incumbent employees.
• Ensures correct laboratory collection, processing and shipping procedures and reschedules missed or insufficient laboratory collections.
• Updates nursing assessment and plan of care in collaboration with the Interdisciplinary Team.
• Educates patient regarding transplant and treatment modality options.
• Participates in education and quality improvement projects at the facility and area level; may serve as Preceptor to new employees.
• Completes CAP requirements to maintain or advance.
• Performs other duties as assigned by Supervisor.

Kenntnisse

Nephrology nursing experience

Ausbildung

Graduate of an accredited School of Nursing

Current state nursing licensure

CPR BLS Certification
