Job summary

Preferred CDCES certification and ongoing commitment to diabetes care; collaboration with the care team to implement comprehensive plans and ensure quality patient outcomes. This role may involve outreach in the Delavan area.

Qualifications

• 2 years of patient education experience.
• Knowledge of DSMES and ADA/ADCES standards.
• Strong communication and presentation skills.
• Ability to tailor education to age-appropriate patients.

Responsibilities

• Lead individual and group education sessions on diabetes management.
• Provide formal instruction on diabetes management.
• Maintain knowledge of diabetes management technology and protocols.
• Collaborate with the healthcare team to implement care plans.
• Document and bill for DSMT and MNT services per CMS.
• Adjust diabetes medications per protocol under supervision.
• Stay current on developments in diabetes care.
• Participate in community events and patient education materials.
• Serve as a resource for providers, patients, and community.
• Complete required hours of CDCES education annually.
