Job summary

Advocate Health in Charlotte, NC is seeking a Diabetes Education Specialist. The role involves leading DSMES initiatives, assessing patient learning needs, and delivering both in inpatient and outpatient diabetes education services.

The educator may provide medical nutrition therapy and coordinate care with the healthcare team. Required is an RN or RD license in the applicable state; CDCES is preferred and certification can be pursued within 2–3 years.

Qualifications

• RN or RD license in applicable state.
• CDCES preferred; certification within 2–3 years if not yet obtained.
• Experience in diabetes education and management is desired.

Responsibilities

• Lead DSMES sessions and provide formal diabetes education to patients.
• Assess learning needs and tailor interventions to individual goals.
• Coordinate care plans and document services per CMS/payor requirements.
• Stay current with ADA/ADCES standards and diabetes care technology.
• Deliver nutrition therapy and follow-up support as applicable.
