Job summary

Advocate Health is seeking a Diabetes Education Specialist to lead DSMT and MNT services for patients with diabetes. The role requires RN or RD licensure, CDCES preferred, and delivering individual and group education aligning with ADA/ADCES standards.

You will collaborate with the care team, document services for CMS and payor requirements, and stay current with diabetes care developments while supporting community education initiatives.

Qualifications

• 2 years of patient-oriented educational experience preferred.
• Awareness of ADA/ADCES standards in diabetes education.
• Current license as RN or registered dietitian; CDCES preferred.
• If not CDCES, certification within two to three years preferred.

Responsibilities

• Lead individual and group education sessions on diabetes management.
• Assess patient learning needs, goals and tailor interventions.
• Collaborate with the healthcare team to implement care plans.
• Document and charge for DSMT and MNT services.
• Stay current on diabetes care developments and technology.
