• Provides education on diabetes management and DSMES per ADA standards.
• Assesses patient needs and develops care plans within policy.
• Educates on diabetes nutrition, exercise, meds, and injectables with proper technique.
• Collaborates with physicians and care teams to adjust treatment as needed.
• Maintains communication about patient plans and educational needs.
