Job summary

Lakeview Hospital seeks a Diabetes Care and Education Specialist to join our team. The DCES will provide direct diabetes education and care, partnering with clinicians to support patients and families across the lifespan.

The role requires RN or RD with diabetes expertise, licensure in WI or MN, and CDCES certification or progress toward it within 24 months. Office work includes Stillwater and Hudson, WI locations with varied scheduling.

Qualifications

• RN or RD with advanced knowledge in diabetes care.
• Experience in diabetes education and care planning.
• Licensure in WI or MN preferred at hire; obtain second license within 30 days if needed.
• Certification in CDCES or ability to achieve within 24 months.

Responsibilities

• Provide direct diabetes education and care.
• Promote self-management and risk reduction.
• Collaborate with care team for integrated, person-centered care.
• Maintain patient confidentiality and rights.
