Job summary

Advocate Health in Wisconsin is seeking a full-time Diabetes Education Specialist to provide DSMES services across inpatient and outpatient settings. The educator will lead individual and group sessions, tailor education to patient needs, and collaborate with the care team to develop comprehensive diabetes care plans.

Candidates must hold RN or Registered Dietitian licensure, preferably CDCES, with 2 years' patient education experience and strong communication skills.

Responsibilities

• Provides DSMES following ADA/ADCES standards.
• Lead individual and group diabetes education sessions.

• Document and bill for DSMT and MNT services.
