Advocate Health is seeking a Diabetes Education Specialist at Lakeshore Southpointe and Sinai locations in Wisconsin. The role focuses on delivering DSMES and MNT services to patients with diabetes, in inpatient or outpatient settings, and involves ongoing education and follow-up.

Candidates should have RN or RD licensure, and CDCES certification is preferred. A Bachelor's degree in Nursing or Dietetics is required, with a Master's in Dietetics preferred if applicable.

Qualifications

• Bachelor's degree in Nursing or Dietetics or related discipline meeting CDCES eligibility.
• RN or RD licensure in the applicable state; CDCES preferred; certification within 2–3 years if not certified.

Responsibilities

• Provide DSMES following ADA/ADCES standards.
• Assess learning needs, goals, and barriers; tailor education.
• Lead individual and group diabetes management education sessions.
