Job summary

The New York Foundling is seeking a Direct Support Professional to empower individuals with developmental disabilities through personalized care and structured activities. You will assist with daily living tasks, meals, medication administration, and transportation while promoting independence and safety.

You will participate in planning and documenting progress, advocate for client rights, and collaborate with healthcare teams to adjust plans.

Qualifications

Responsibilities

Implement and monitor care plans, including Comprehensive Functional Assessments, Life Plans, and Behavior Support Plans.
Provide direct care: daily living activities, personal hygiene, meals, medication administration, transportation.
Facilitate community outings and social events to promote inclusion and independence.
Use person-centered approaches and positive behavior support strategies to address behaviors.
Maintain accurate records, progress notes, and incident reports in compliance with policies.
Collaborate with interdisciplinary teams to develop and implement holistic care plans.
Advocate for rights, choices, and self-advocacy; act as custodian and mandated reporter per policy.
