Job summary

The New York Foundling is seeking a Direct Support Professional to empower individuals with developmental disabilities to live fulfilling lives. You will implement and monitor plans of care, assist with daily living activities, medication administration, transportation, and documentation, while promoting independence and social skills.

Qualifications

• High school diploma or GED; experience in direct care or human services preferred.
• Valid driver’s license with 6 months of experience and 6 or fewer points.
• CPR and First Aid certification preferred; additional training in developmental disabilities, behavior management, or healthcare a plus.
• Commitment to compassionate care and promoting dignity and respect for individuals with developmental disabilities.
• Ability to respond to changing needs and environments with flexibility and problem-solving.

Responsibilities

• Responsible for implementing and monitoring plans of care for the persons supported.
• Provide direct care and assistance in daily living activities including personal hygiene, meals, transportation, and medication administration.
• Facilitate community outings and social activities to promote inclusion and independence.
• Provide supervision and support for medical, developmental, clinical, or emotional needs as required.
• Maintain records, progress notes, and incident reports per policies.
• Collaborate with interdisciplinary teams to develop comprehensive care plans.
• Advocate for rights and choices, promote self-advocacy, and adhere to confidentiality and safety standards.
