Job summary

The New York Foundling seeks a Direct Support Professional to empower individuals with developmental disabilities to live fulfilling lives. You will implement care plans, document progress, and provide daily living assistance in a supportive setting.

Responsibilities include medication administration, transportation, and facilitating community involvement to promote independence and well-being.

Qualifications

• High school diploma or equivalent required.
• Valid driver’s license with 6 months of experience and 6 or fewer points.
• Strong interpersonal and communication skills; patience, empathy, and a genuine desire to support persons with developmental disabilities.
• CPR and First Aid certification preferred; additional training in developmental disabilities, behavior management, or healthcare a plus.

Responsibilities

• Implement and monitor plans of care for persons supported, including assessments, Life Plans, and behavior support plans.
• Provide direct care such as daily living activities, personal hygiene, meals, medication administration, and transportation.
• Facilitate community outings and social activities to promote inclusion and independence.
• Use person-centered approaches to address behaviors and ensure safe environments.
• Maintain accurate records, progress notes, and incident reports per agency policy.
• Collaborate with healthcare professionals, therapists, and family to develop comprehensive care plans.
• Advocate for rights and choices of individuals, supporting self-advocacy and decision-making.
• Be a custodian and mandated reporter as required by OPWDD, NYSJC, and policies.
