Qualifications

• High school diploma or equivalent required; experience in direct care preferred.
• Strong interpersonal and communication skills; patience and empathy.
• Ability to respond flexibly to changing needs.
• CPR and First Aid certification preferred; valid driver’s license recommended.

Responsibilities

• Implement and monitor care plans including assessments and life plans.
• Provide direct care: daily living activities, medication administration, transportation.
• Facilitate community activities and accompany appointments.
• Maintain records and incident reports per policy.
• Collaborate with healthcare professionals and family members.
• Advocate for rights and promote self-advocacy.
• Be a custodian and mandated reporter per policies.
