Job summary

The New York Foundling is seeking a Direct Support Professional to empower individuals with developmental disabilities in a supportive, growth-focused environment. You will implement plans of care, provide daily living assistance, and support independence through structured activities.

Responsibilities include documenting progress, supervising activities, and coordinating with healthcare teams to ensure holistic care, safety, and respect for client dignity in NY settings.

Qualifications

• High school diploma or GED required.
• Valid NY/NJ/CT/PA Driver’s License with 6 months of experience and 6 or less points.
• Strong interpersonal and communication skills; patience, empathy, and a genuine desire to support persons with developmental disabilities.
• CPR and First Aid certification preferred; additional training in developmental disabilities or healthcare is a plus.
• Commitment to providing compassionate care and promoting dignity and respect for individuals with developmental disabilities.
• Ability to respond effectively to changing needs and environments, demonstrating flexibility and problem-solving skills.
• Maintain confidentiality, uphold ethical standards, and adhere to agency policies and procedures.

Responsibilities

• Implement and monitor plans of care, such as assessments, life plans, and behavior support plans.
• Provide direct care, including daily living activities, personal hygiene, meal prep, medication administration, and transportation.
• Facilitate community outings and social events to promote inclusion and independence.
• Use person-centered approaches and positive behavior support strategies to address challenging behaviors.
• Maintain records, progress notes, and incident reports in line with policies and regulatory requirements.
• Collaborate with healthcare professionals, therapists, and family members to develop care plans.
• Advocate for rights and choices of individuals and act as a custodian and mandated reporter as per policy.
