Job summary

BrightPath LLC is seeking a Designated Coordinator to support person-centered community-based services for adults with disabilities in the Twin Cities metro. This role blends program coordination, staff supervision, direct service, and Minnesota 245D compliance.

You will collaborate with individuals served, guardians, case managers, and providers to implement individualized service plans, maintain documentation, and lead staff development while ensuring high-quality outcomes.

Qualifications

• Requires Minnesota 245D Designated Coordinator qualifications via one of four pathways (degree or diploma with direct care experience).
• Experience supervising staff and coordinating services in a human services setting.
• Strong written and verbal communication skills and ability to manage multiple responsibilities.
• Valid driver’s license, reliable transportation, and ability to meet driving requirements.

Responsibilities

• Coordinate services for individuals receiving community-based supports.
• Conduct client intakes, admissions, assessments, and service planning activities.
• Develop, implement, review, and update individualized service plans.
• Attend support team meetings and liaise with guardians, case managers, and providers.
• Maintain client service agreements, reports, and program documentation.
• Support incident response and required reporting.
• Ensure services are person-centered and aligned with goals.
