Job summary

BrightPath is seeking a Designated Coordinator to support delivery of person-centered community-based services for adults with disabilities. This hands-on leadership role blends program coordination, staff supervision, direct service, and Minnesota 245D compliance.

You will work closely with individuals served, families, and the care team to ensure plans are implemented, incidents are reported, and services stay aligned with each person’s Coordinated Service and Support Plan.

Qualifications

• Bachelor’s degree in a human services field with 1 year of direct care experience.
• Associate degree in human services with 2 years of direct care.
• Postsecondary diploma in human services with 3 years direct care.
• Or a combination meeting Minnesota 245D requirements.

Responsibilities

• Coordinate services for individuals receiving community-based supports.
• Conduct intakes, admissions, assessments, and service planning activities.
• Develop and update individualized service plans.
• Attend meetings and liaise with guardians, case managers, and providers.
• Maintain client agreements, reports, and program documentation.
• Support incident reporting and ensure services are person-centered.
• Lead staff supervision and development to ensure compliant service delivery.
