Royal Crown Homes LLC in Bloomington, MN seeks a dedicated Designated Coordinator to support individuals with disabilities, coordinating ISPs, planning meetings, and ensuring high-quality care.

You will supervise DSPs, maintain records, and ensure compliance with DHS 245D standards, working with families, case managers, and professionals to advocate for residents.

Qualifications

• Experience in residential services or disability services.
• Knowledge of Minnesota 245D licensing requirements.
• Designated Coordinator qualifications as defined by Minnesota Statute.
• Strong person-centered planning and ISP development skills.
• Excellent communication and documentation abilities.
• Ability to pass a Minnesota DHS background study.

Responsibilities

• Develop, implement, and monitor individualized service plans (ISPs) for individuals receiving residential support services
• Serve as the primary point of contact for individuals, families, case managers, and other stakeholders
• Coordinate and facilitate person-centered planning meetings and reviews
• Ensure compliance with DHS licensing standards, 245D regulations, and all applicable state and federal guidelines
• Supervise and support direct support professionals (DSPs) to ensure quality care delivery
• Maintain accurate documentation, progress notes, and service records
• Conduct assessments and identify support needs, goals, and outcomes for each individual served
