Job summary

MN Care Services is seeking a Designated Coordinator to organize respectful, individualized support for people in home-based settings. You will supervise staff, coordinate schedules, and ensure accurate documentation while prioritizing safety and person-centered care.

Responsibilities include team meetings, data collection, care-plan adherence, and transportation. A valid driver’s license and clear background are required; pay ranges from $50,000 to $65,000 per year with benefits.

Qualifications

• Bachelor’s degree in psychology, social work, human services, or healthcare.
• Associate’s degree with 2+ years of direct experience in behavioral/mental health settings.
• High school diploma with 3+ years of progressive experience in relevant settings.
• Valid driver’s license with clean driving record.
• Ability to pass MN DHS background study (fingerprinting) and 245D training.

Responsibilities

• Provide supervision, training, and support for worksite staff.
• Conduct monthly team meetings to ensure consistent communication.
• Maintain staff schedules at least two weeks in advance.
• Review timesheets and approve time-off requests promptly.
• Ensure medication is ordered and MAR reviews completed with Nursing staff.
• Document care plans, shift notes, and medication administration reports timely.
• Collect data to drive positive behaviors per the person’s care plan.
• Support activities of daily living and community engagement for individuals.
