Zusammenfassung

Endeavors is seeking a Disaster Human Services Case Manager to provide survivor-centered case management during field or remote operations in the Austin, TX area. You will conduct intake and screening, assess needs, develop recovery plans, and coordinate resources while protecting privacy and promoting equitable access to services.

Required duties include documenting progress, supporting deployment readiness, and following FEMA guidance.

Qualifikationen

• Bachelor's degree from an accredited college or university is preferred.
• Equivalent education and experience may be considered.
• Experience in case management or human services is preferred.
• Disaster recovery experience is strongly preferred.

Aufgaben

• Conduct disaster survivor intake, screening and needs assessments with approved tools and procedures.
• Develop individualized recovery plans with survivors, set goals and adjust plans as needs change.
• Identify and coordinate available federal, state, tribal, local, nonprofit resources to address unmet needs.
• Provide referrals and follow-up; help survivors navigate benefits and recovery resources.
• Maintain survivor records, progress notes and required statistics in case management systems.
