Qualifications

  • Conduct community outreach, workshops, trainings, and presentations for individuals and service partners.
  • Maintain accurate documentation, case records, reports, authorizations, and service delivery data.
  • Proficiency in American Sign Language.
  • Strong Deaf culture understanding and appreciation.
  • Bachelor’s degree from an accredited college or university.
  • At least 2 years of experience serving d/Deaf, DeafBlind, hard of hearing, or related populations.
  • Experience in advocacy, social services, or independent living fields.
  • Knowledge of resource navigation and community-based services.
  • Ability to handle stressful situations with professionalism and empathy.
  • Strong computer skills for confidential data entry and records.

Responsibilities

  • Maintain an active caseload of ~30 consumers with advocacy, case management, and independent living skills training.
  • Assist clients with housing, employment, benefits, healthcare, transportation, and community resources.
  • Navigate systems like DOR, Regional Center, Medi-Cal, CalAIM, housing programs, and employment services.
  • Develop and deliver individual and group workshops and outreach activities.
  • Maintain accurate case notes, consumer records, and billing data in compliance with HIPAA.
  • Build relationships with community organizations, employers, housing providers, shelters, and agencies.

Job Summary

A mission-driven nonprofit organization in Anaheim, CA is seeking a Deaf Services Case Manager for a direct hire opportunity supporting individuals who are d/Deaf, DeafBlind, hard of hearing, DeafDisabled, or late-deafened. This role is ideal for a compassionate advocate who is fluent in American Sign Language and passionate about accessibility, independent living, and helping people overcome barriers to essential services.

Key Responsibilities
  • Maintain an active caseload of approximately 30 consumers while providing individualized advocacy, case management, peer support, and independent living skills training.
  • Assist consumers with accessing housing, employment, benefits, healthcare, transportation, legal, and community-based resources.
  • Help individuals navigate public and social-service systems, including DOR, Regional Center, Medi-Cal, CalAIM, housing programs, and employment services.
  • Develop and deliver individual and group workshops, trainings, presentations, and outreach activities related to accessibility, self-advocacy, tenant rights, employment, and independent living.
  • Maintain accurate case notes, consumer records, reports, billing documentation, and service data in accordance with confidentiality, agency, and HIPAA requirements.
  • Build relationships with community organizations, employers, housing providers, shelters, healthcare providers, government agencies, and other service partners.
Compensation and Benefits
  • $23.00 to $26.00 per hour.
  • Direct hire opportunity.

Required Qualifications and Skills

  • Proficiency in American Sign Language.
  • Strong understanding and appreciation of Deaf culture and the Deaf community.
  • Bachelor’s degree from an accredited college or university.
  • At least 2 years of experience working with individuals who are d/Deaf, DeafBlind, hard of hearing, DeafDisabled, or in a related field.
  • Experience in advocacy, case management, social services, independent living, community services, or a related area.
  • Strong knowledge of resource navigation, consumer support, accessibility, and community-based services.
  • Ability to support individuals through stressful or challenging circumstances with professionalism, empathy, and appropriate boundaries.
  • Strong computer skills with the ability to accurately enter, maintain, and protect confidential information.
  • Excellent organizational, communication, multitasking, problem-solving, and follow-through skills.
  • Ability to work independently and collaboratively while managing multiple priorities.
  • Valid California driver’s license, automobile insurance, and reliable transportation.
Preferred Qualifications
  • Knowledge of DOR, Regional Center, CalAIM, Medi-Cal, housing programs, employment services, and other social-service systems.
  • Understanding of housing resources, tenant rights, and related legislation.
  • Knowledge of the Independent Living philosophy and methodology.
  • Experience building community partnerships with employers, housing providers, shelters, healthcare organizations, government agencies, and nonprofit service providers.
  • Experience developing and facilitating workshops, trainings, presentations, or outreach events for diverse community audiences.
