Avamere at Chestnut Lane, a Deaf and Deaf-Blind community in Gresham, OR, seeks a Support Services Provider to bridge communication barriers for residents requiring sign language access and advocacy for independent living.

The role emphasizes coordinating interpreters, creating individualized support plans, and helping residents age in place with culturally accessible services, while maintaining HIPAA compliance and empathy for seniors.

Qualifications

• Prefer bachelor’s degree in special education, social work, counseling or related field.
• Exceptional fluency in American Sign Language (ASL) and familiarity with tactile sign language, Pro-Tactile, and diverse deaf-blind communication systems.
• Minimum of 1–3 years of relevant professional experience with elderly individuals or disabled populations, preferably Deaf/Deaf-Blind communities.
• State certification (e.g., SSP) and active CPR/BLS during employment.
• Excellent verbal and written communication with residents, families, and staff.
• High empathy and understanding for older adults’ needs; strong multitasking and confidentiality adherence.

Responsibilities

• Relays information using residents’ preferred communication modes (ASL, tactile, braille).
• Coordinate sign language interpreters, captioning, and augmentative communication for appointments and meetings.
• Develop, monitor, and update individualized support plans with residents and staff.
• Connect residents to outside agencies and resources; assist with transportation and daily living activities.
• Advocate for residents’ rights under ADA and ensure culturally accessible services.
