Job summary

Qualifications

- Experience monitoring service plans and coordinating with residential/housing settings.
- Knowledge of federal/state regulations for case management in DD services.
- Experience collaborating with community agencies or law enforcement.
Responsibilities
- Plan, assess, and document program services per Oregon Administrative Rules (OAR).
- Conduct home visits to assess needs and complete annual service plans.
- Coordinate entry, exit, and transfer meetings for in-home and residential services.
- Monitor service payments and ensure compliance with plans.
- Provide crisis management and inter-agency coordination.
