Job summary

Clackamas County Social Services seeks Case Managers to support individuals with intellectual and developmental disabilities through community-based services. You will authorize and monitor services, coordinate residential supports, and develop plans with a focus on dignity and belonging.

Typical tasks include home visits, service plan development, and cross-department coordination to ensure compliance with Oregon rules and funding mechanisms, while promoting equity and person-centered care.

Qualifications

• Three years of human services related experience or an associate's degree with two years of experience or a bachelor’s degree with one year of experience.
• At least 1 year of current DD services experience and case management activities.
• Experience collaborating with community agencies and understanding relevant regulations.

Responsibilities

• Participate in planning, authorization of program services per OAR rules.
• Conduct home visits to assess needs and complete annual service plans.
• Coordinate meetings for individuals in home/residential/employment services.
• Monitor needs assessments and document interactions in required systems.
• Ensure eligibility and payments and coordinate with Medicaid billing via Plan of Care system.
