You will coordinate services for up to 30 individuals with I/DD, implement person-centered planning, and ensure quality services across the program. The role requires a Bachelor’s degree or related experience, strong interpersonal skills, and the ability to manage multiple tasks with discretion.

Qualifikationen

Aufgaben

• Supervise Rehabilitation Specialists and Direct Support Professionals and ensure schedules are followed.
• Coordinate services for up to 30 individuals with I/DD and participate in person-centered planning.
• Complete required documentation (Life Plans, Monthly Case Notes, Plans, Assessments) on time.
• Visit sites and monitor service quality, safety, and interactions.
• Hire and discipline staff with oversight from QA and program management.
