Job summary

Goodwill NYNJ is seeking a dedicated professional to coordinate Prevocational and Day Habilitation services for up to 30 individuals with Intellectual Disabilities. You will engage in Person-Centered Planning, develop Habilitation Plans, and ensure documentation meets funding sources' and agency standards.

You will supervise Direct Support Professionals, visit program and volunteer sites, balance petty cash, process transportation reimbursements, and support scheduling and progress toward

Qualifications

• Bachelor's degree in a related field required.
• One year of work experience in mental health, vocational rehabilitation, or case management preferred.
• Strong interpersonal and written communication skills, with ability to handle confidential information discreetly.

Responsibilities

• Coordinate Prevocational and Day Habilitation services for up to 30 individuals with ID.
• Complete required documentation (Habilitation Plans, Monthly Case Notes, etc.) to standards.
• Oversee DSP implementation of services and ensure timely reporting and planning.
• Develop and maintain Person-Centered Plans and Habilitation Plans.
• Supervise activities at hub sites and offsite locations, ensuring quality and progress.
