Job summary

Sevita operates a Mentor Adult Day Health program in West Boylston, MA, seeking a compassionate Program Assistant to support elders with complex medical or rehabilitative needs. The role offers day shift hours at $16.00/hour in a community-based setting.

You will assist with daily living activities, lead group activities, monitor safety, and help participants maintain independence. A caring, detail‑oriented team player is essential.

Qualifications

• High school diploma or equivalent; must be 18+ years old.
• Minimum 1 year experience with individuals with intellectual or developmental disabilities, preferably with older adults.
• CPR and First Aid certification.

Responsibilities

• Provide daily care to health-impaired older adults and individuals with disabilities.
• Assist with activity programs and social/recreational activities for participants.
• Help with daily living tasks: hygiene, bathing, grooming, meals, and medication administration.

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Program Assistant – Adult Day Health

West Boylston, MA

Day Shift | $16.00/hour

What You'll Do

• Provide assistance in the daily care of health-impaired older adults or individuals in our care with brain injuries, intellectual/developmental disabilities, emotional or behavioral disabilities, or mental health diagnoses.
• Assist with the implementation of individual and group activity programs, and be responsible for the care, safety, security, and social/recreational activities for the individuals receiving support.
• Assist individuals with daily living, including personal hygiene, bathing, bathroom assistance, grooming, meal preparation, and medication administration.
• Provide guidance with social skills, advocacy, and self-esteem, and teach strategies to live as independently as possible.

Qualifications

• High school diploma or equivalent; must be 18 years of age or older
• Minimum one year of experience providing services to individuals with intellectual and developmental disabilities, preferably with older adults
• Proficiency in vital signs, weights, Activities of Daily Living (ADLs), and independent ADLs
• Competence in CPR and First Aid
• Strong attention to detail and effective communication skills
• A reliable, responsible attitude and a compassionate approach
