Job summary

Hope-Haven-Inc. seeks a Day Lead to guide staff in a Day Habilitation program, focusing on compassionate, client-centered support and learning. You will coordinate daily activities, transportation, and community engagement while ensuring compliance with Medicaid rules.

The role emphasizes teamwork, documentation accuracy, and continuous improvement in client well-being within a Monday–Friday schedule.

Qualifications

• High school diploma is required.
• Two years' experience working with people with developmental disabilities.
• Strong behavior management skills and ability to document.
• Proficient computer use and word processing.
• Knowledge of assistive technology and adaptive equipment.
• Valid driver’s license and insurance coverage.

Responsibilities

• Facilitate calendar planning meetings.
• Apply person-centered planning; tailor activities to individual needs.
• Coordinate daily Day Habilitation activities and staff support.
• Arrange transportation for Day Habilitation activities.
• Support community integration and community life participation.
• Provide input on service objectives and procedures.
• Complete daily documentation compliant with Medicaid waiver rules.
• Ensure data accuracy for Performance Measurement System.
